Throughout your study, Flute & Friends Suzuki Music Academy (the "Studio"), may wish to interview, photograph or record (audio and/or visual) your child (or yourself, if you are the student) for promotional, instructional, and educational reasons to utilize in brochures, publications, posters, newsletters, social media and the Flute & Friends Suzuki Music Academy website.

Before you (if adult), or your child (if a minor), can participate in any of the above activities, you must indicate which of the following categories of media you give your permission for potential use of your/your child's image, video recording, audio recording, or name ("Use of Personal Image"). Thank you for your cooperation and understanding.
